A tire warehouse worker was using a forklift when he fell about 3.6 metres to the ground. He died three days later from his injuries. A CSST investigation found that there were inadequate protections against falls from a forklift and the layout of the warehouse forced workers to use a method that exposed them to a fall hazard.
