Public health guidelines in some jurisdictions now require employers to not only perform pre-entry COVID-19 screening but also confirm that workers and others seeking entrance have performed a daily health check on themselves. Here’s a form you can use to obtain and document that you got the necessary confirmation. Caveat: This form is based on public health guidelines in effect in February 2021, which are subject to change at any time and may vary by jurisdiction, particularly with regard to the travel item listed first. Thus, in some jurisdictions, the risk factor is not travel outside Canada but travel outside the province.
