Bloodborne Pathogens: Hazard Assessment Worksheet Template

Hazard identification and assessment is the first step in controlling bloodborne pathogens hazards. While hazard assessment methods vary, it must be performed by a competent person with knowledge of BBPs and the operations carried out. At a minimum, it should evaluate:

  • Particular jobs or tasks that may involve BBP exposure hazards;
  • The specific hazards posed;
  • A characterization of whether those hazards pose high, medium, or low risk;
  • The current controls in place to manage those hazards, including engineering controls, work/administrative controls, and PPE;
  • Additional measures needed to control the hazards identified; and
  • The person responsible for implementing those measures.
