Workplaces where workers use equipment and machinery and so are at risk of injury when repairing and maintaining that equipment should have lockout/tagout (LOTO) procedures. These procedures typically require equipment to be turned off and rendered inoperable with the use of a lock before workers clear jams, make repairs, etc. Workers who perform such hazardous tasks must be trained on your LOTO procedures—and you must be able to prove that you provided such training and what the training covered.
HOW TO USE THE TOOL
Use this form when providing LOTO training to workers. Adapt it for your company, OHS program and the requirements in your jurisdiction’s OHS regulations. Ensure that it’s completed after each LOTO training session and retain the forms to document your training.
