After two workers on a fishing vessel were sent to clean the shrimp tank, one flipped a switch that closed a hydraulic door, crushing the other to death. The worker who turned on the machinery said he was never instructed on how to stop the doors in an emergency. The fishing company pleaded guilty to two safety violations, including failing to provide necessary safety training, and was fined $90,000 [Katsheshuk Fisheries, Aug. 25, 2014].
