A meat plant worker lost three fingers while cleaning a machine. The government charged the employer and a supervisor with violations of the OHS Act, claiming that the employer failed to ensure its workers and supervisors were familiar with hazards at the workplace and that each piece of equipment was used according to safe work practices.
