The owner of a roofing company pleaded guilty to three OHS violations, including failing to ensure that a ladder in use by workers was secured; to ensure workers used a fall protection system; and to have a written fall protection plan. The charges were laid after a proactive routine inspection by an Occupational Health Officer, who saw four workers shingling a roof without fall protection [Advanced Roofing, Govt.
