Oct. 22: Supervision of Trainees Criticized in TSB Incident Report

The Transportation Safety Board of Canada (TSB) released its report into a Nov. 2013 fatal rail accident in Saskatchewan. While performing switching operations, a CN freight train struck and seriously injured a conductor trainee, who was securing a derail mounted between the rails. The trainee died on the way to the hospital. The TSB investigation identified lack of direct supervision of conductor trainees and misapplication of safety critical operating procedures as contributing factors in the incident.
