March 3: TSB Report Criticizes Emergency Planning on Fishing Vessel

The Transportation Safety Board of Canada released its investigation report on an incident in which the master of the fishing vessel Four Ladies 2003 died after stacked lobster traps fell onto the main deck and knocked him overboard. The investigation identified risks with respect to emergency preparedness. For example, the crew members didn’t have an emergency response plan to follow and hadn’t practiced retrieval of a man overboard during drills.
