A worker was using a machine to pull a wire through a duct when his fingers got caught in the machine’s pulley system. As a result, several of the worker’s fingers were partially crushed. An MOL investigation found that there were no guards on the machine to prevent a worker’s fingers from getting caught. The company pleaded guilty to a guarding violation and was fined $55,000 [Canadian BBR Inc., Govt. News Release, April 15, 2010].
