Feb. 9: Transportation Safety Board Releases Report on Helicopter Crash


The Transportation Safety Board today released its final report on the fatal 2009 crash of Cougar Flight 91 off the coast of NL. Key recommendations:

  • Removal of the “extremely remote” provision from the rule requiring 30 minutes of safe operation following the loss of main gearbox lubricant

  • Assessment of the adequacy of the 30 minute main gearbox run dry requirement for Category A transport helicopters

  • Ban on commercial operation of Category A transport helicopters over water when the state of the sea won’t permit safe ditching and successful evacuation

  • Requirement that supplemental underwater breathing apparatus be mandatory for all occupansts of helicopters on overwater flights who must wear Passenger Transportation Suit Systems.
