Fatality. One of the firm’s workers sustained fatal head injuries when he fell approximately 8.8 m (29 ft.) from the roof of a three-storey residential townhouse complex. The worker had been wearing a fall protection harness, but had not been secured to an anchor system. The worksite had no site-specific fall protection plan. The officer found that the firm failed to provide its workers with the information, instruction, training, and supervision necessary to ensure their health and safety while carrying out their work [Jas Construction Ltd., Feb. 23, 2009].
