A worker at a manufacturing facility was operating a production line when he noticed a defect in the product coming out of the line. He tried to wipe a blade, which was attached to a pair of rollers that guided the product through the line. But his hand was drawn into the machine by the rollers and injured. An MOL investigation found that the blade and rollers weren’t guarded. The company pleaded guilty to a guarding violation and was fined $55,000 [3M Canada Co., Govt. News Release, March 3, 2011].
