Respiratory Hazard Assessment Form

BENEFITS

If there are respiratory hazards, such as gases or dust, in your workplace or related to a work activity, you have a duty to protect workers from them. To do so, you first have to assess those hazards. A written assessment of respiratory hazards is important to determine if respiratory protection is needed and, if so, to select of the correct type of protection.

HOW TO USE THE TOOL

Adapt this respiratory hazard assessment form for the OELs and respiratory protection requirements in your jurisdiction’s OHS laws. It should be completed by a supervisor or other ‘competent’ person whenever there’s a chance workers could be exposed to fumes, toxic gases, particulates, low oxygen levels, etc. Use the results of the assessment to determine the appropriate respiratory protection for workers exposed to the identified hazards.
