Workers who are new to the workplace and those who are under age 25 are the most vulnerable to injury on the job. Their lack of experience and reluctance to ask questions contribute to their vulnerability. Implementing a safety mentor program is one thing you can do to better protect them. To ensure that your mentor program is effective, have mentors and mentees complete an evaluation form when the mentorship is over.

Adapt this model evaluation form for your workplace and its mentor program. It’s designed for use by both mentors and mentees but you could also create separate versions for each. Use the feedback from these completed forms to evaluate the effectiveness of your mentor program and implement changes or improvements.
