Lockout Violation Costs Employer $50,000

A worker was testing equipment when it malfunctioned. He tried to fix the problem and accidentally activated the equipment, which resulted in his arm being pinned and injured. The MOL investigation found that the equipment hadn’t been checked to find out if it was operational and the electrical source wasn’t locked out as required.
