Lock-out Violations Cost Employer $7,800

A worker’s hand was crushed when it got jammed between a strapper and a metal post. His employer pleaded guilty to failing to establish a written lock out procedure for a machine and ensure that a worker who may have to lock out a machine has been adequately trained to do so.
