The Toronto Transit Commission (TTC) released its final investigation report on the death of a roadmaster and supervisor, who was struck and killed by a work car on subway tracks. Two other workers suffered injuries. The supervisor and a track mechanic were inspecting track defects beyond the bounds of an established work zone but hadn’t requested permission to conduct a walking inspection as required. So the control centre didn’t alert work car operators of their presence. The report also includes an action plan for preventing similar incidents.
