Nov. 24: Two workers were securing roof trusses when they gave way after a brace was removed, causing the workers to fall. A 72-year-old worker was killed and his co-worker injured. A coroner’s inquest found several safety lapses, including the failure of the workers to wear fall protection. In addition, building inspectors had been to the site a dozen times but reported no safety violations. The coroner’s jury recommended that building inspectors and field engineers get basic OHS training to be able to identify potential safety violations.
