May 9: Inquiry into Deaths at Mushroom Farm Begins
A coroner’s inquest into the deaths of three workers at a mushroom farm began. Two workers were overcome by toxic gases after they entered a compost shed to clear a clogged pipe and three others rushed in to rescue their co-workers. Two companies, along with their three owners, were fined $350,000 after pleading guilty to 10 OHS violations. The evidence so far has indicated that language barriers were an issue and that the farm didn’t have a JHSC or safety policy manual.
