The Transportation Safety Board of Canada (TSB) released its investigation report into the Aug. 2011 fatal crash of First Air flight 6560 in Resolute Bay. The charter flight struck a hill about one nautical mile east of the runway, killing all four crew members and eight of the passengers, and seriously injuring three other passengers. A combination of factors contributed to the accident. For example, although the pilots knew they were off course, they each had a different understanding of the situation and didn’t take corrective action in time. The TSB believes that stronger measures to improve crew communications are needed.
