July 19: Crew Fatigue Was a Factor in Train Derailment
The TSB released its investigation report on the derailment of a CN Railway train near Kwinitsa, BC. There were no injuries and no dangerous goods were involved. The TSB’s investigation determined that a seized and locked axle, crew fatigue and misinterpretation of a fault indicator led to the incident. As to fatigue, in the preceding days, the crew members had erratic sleep patterns due to work shifts with variable start and end times. Such work/sleep patterns cause circadian rhythm disruptions, which can decrease performance and cognitive function.
