The Chief Inspector of Mines completed his investigation into the tailings storage facility breach at Mount Polley and released his final report, which contains the process and findings of the investigation and 19 recommendations, such as:
- Any mine with tailings storage facilities should have a qualified individual designated as a mine dam safety manager, who’s responsible for oversight of planning, design, operation, construction and maintenance
- All mine personnel should be educated in the recognition of conditions and events that could impact tailing storage safety or violate applicable permit conditions and regulations.
