The Transportation Safety Board released its investigation report into a plane crash that killed seven people in Québec City. The TSB found numerous safety deficiencies. For example, while the crew’s training met regulatory requirements, it didn’t prepare them to effectively manage emergencies. Also, an emergency checklist was designed for use by a single pilot and didn’t specify who was to perform which tasks during two-pilot operations. The report points to the airline’s poor safety culture as responsible for many of the failures.
