A construction worker fell off scaffolding and broke his wrist, hip and ankle. He was one of only two construction workers working at the site of an expansion project—and both were only in their second week of work with the company. An investigation found that the rolling scaffold being used hadn’t been fitted with guardrails and the wheels weren’t locked.
