During an OHS inspection at a construction site, a worker was seen at the bottom of an unshored trench. As a result, the city pleaded guilty to failing to ensure that a worker was protected from cave-ins or sliding material in a trench and that a trench that a worker was required or permitted to enter was kept free from an accumulation of water. The court fined it $8,000 [The City of Meadow Lake, Govt. News Release, Feb. 28, 2014].
