A crew for a roofing company removed heating, ventilation and air conditioning cones and other roofing material from the roof of an industrial establishment. The next morning, a worker fell through an opening in the roof and 28 feet to the concrete floor below, sustaining serious injuries. The company had failed to ensure that the worker was adequately protected by a guardrail system. It pleaded guilty and was fined $57,500 [Paiva Roofing Ltd., Federal Govt.
