At a retailer’s warehouse, a worker was standing in front of a cardboard compactor’s discharge door trying to eject a bale of cardboard when the door suddenly flew open. He was hit by the door and pinned against a steel column, suffering multiple fractures. The MOL found that there was no interlock system in place to prevent the machine from working while the discharge door was being opened. The company pleaded guilty to failing to have such a system and was fined $55,000 [Holt Renfrew & Co. Ltd., Govt. News Release, Jan. 13, 2012].
