Two city workers were observed in a trench with straight cut sides and water seepage in the bottom. The walls of the excavation site didn’t comply with OHS requirements. The city pleaded guilty to violating the OHS Act by failing to ensure that workers were protected from cave-ins or sliding materials in a trench and that all work was sufficiently and competently supervised. The court fined it $6,180 [City of Meadow Lake, Govt. News Release, Aug. 7, 2012].
