The OHS laws require employers to provide various kinds of safety training to workers, from initial safety orientations to training on specific hazards and safe work procedures. But how do you know if your safety training is effective and that workers are getting all that they should out of it?

Adapt this form for your safety training programs. Give it to workers after each training session and ask them to complete it. Use the feedback from the evaluation forms to improve your training. (Note: It’s a good idea to allow workers to fill out the forms anonymously to encourage candid feedback.)
