Guarding and Supervision Violations Result in $10,000 Fine

A worker was seriously injured when he became entangled in the drive shaft of a conveyor while crushing gravel. His employer pleaded guilty to failing to provide an effective safeguard where a worker may contact a dangerous moving part of a machine and to ensure that all work at a place of employment was sufficiently and competently supervised.
