Death of Worker Due to Lack of Guard Costs Company $16,800

A worker was killed while installing culverts when he was dislodged from a piece of machinery that rolled onto him. His employer pleaded guilty to one OHS violation, failing to ensure that an effective safeguard was in place to prevent contact with a dangerous part of a machine.
