Confined Space Hazard Assessment Form


Of all of the hazards in the workplace, confined spaces are among the most dangerous—for both workers who enter such spaces and rescuers coming to the aid of workers in trouble. To comply with OHS requirements for confined spaces, you first need to identify the “confined spaces” in your workplace and then assess the hazards in those spaces you’ve identified.


Adapt this confined space hazard assessment form, which was based on one created by WorkSafe Alberta, to be consistent with your company’s operations and other workplace safety policies and with the requirements in your jurisdiction’s OHS laws on confined spaces. A “competent person” should use it to assess the possible hazards in any confined space in your workplace before workers enter it. Once the hazards have been identified, take appropriate steps to address them.
