City Fined $80,000 for Worker’s Fall from Truck

A worker at a public works yard was preparing a truck to haul gravel by removing a large grate on top of the truck box and hooking it to a chain attached to a backhoe. He was on the grate and moving toward a fixed ladder to get to the ground when the grate shifted, causing him to fall to the ground. He suffered cracked bones, a dislocation and bruising. The city pleaded guilty to an OHS violation and was fined $80,000 [The City of Ottawa, Govt. News Release, May 13, 2013].
