Here’s a model policy explaining what to list in an accident report. This should be part of a larger policy that explains when accidents must be reported, to whom accident reports should be sent and how long after accidents occur reports must be filed. Make sure your accident reporting policy follows the requirements of your province or territory’s (or federal) OHS laws. This is especially important when it comes to listing what information to list in the accident report.
