Workers were burning waste from the manufacture of pyrotechnics by taking large bags of waste from a truck, repackaging them into smaller bags and placing those bags onto a burn pit. One of the bags on the truck ignited, setting the truck on fire. Two workers were burned. The company pleaded guilty to failing to establish a written procedure for the disposal and destruction of pyrotechnic waste and was fined $50,000 [HFI Pyrotechnics Inc., Govt.
